Go to franchise settings & turn progressive fatigue off, then gameplay settings and fatigue slider down to zero. Last go to manage staff -> auto sub: turn ALL “sub out” sliders all the way to 0 so your starters aren't subbed out. Last step is what fixed it for me.

If Progressive Fatigue and Auto Sub are both enabled the players with high fatigue might be automatically getting subbed with the backups. You should be able to disable Progressive Fatigue and set all the sub sliders to 0 under Auto Sub in the Franchise settings.

The game is recognizing that your players are too tired. and therefore not putting them in the game. So the first and simplest way to fix this is that. anytime you start by franchise, make sure progressive fatigue is set to off.
